Full‑Time Nurse Navigator – Patient Care Coordination

About the role

The Department of Medicine, Division of Cardiovascular at the University of Miami seeks a full‑time Nurse Navigator 1 (H) to serve as the first point of contact for patients entering the health system. The role ensures timely scheduling, coordinates care across the continuum, and acts as a liaison among patients, families, and the multidisciplinary team.

Key responsibilities

• Triages new patient appointments to appropriate providers and ensures timely initial scheduling.
• Reviews external medical records and orders necessary testing before appointments.
• Assesses barriers to care and refers patients to support services and community resources.
• Educates patients on treatment plans and supports them throughout the care continuum.
• Collaborates with a multidisciplinary team to determine optimal treatment pathways.
• Mentors students, new graduates, and visiting professionals.
• Maintains professional knowledge through continuing education and professional organization involvement.
• Acts as liaison between patients and providers, entering orders for cardiac monitors, echocardiograms, stress tests, coronary CTA, labs, and referrals.

Required skills

• Familiarity with coronary CT angiography (CTA) procedures.
• Ability to order and manage laboratory tests.
