About the role

Exceptional care starts with exceptional people. This position provides direct patient care, including IV therapy, for assigned patients in a Skilled Nursing Facility under the Licensed Practical Nurse Act and the direction of a Registered Nurse.

Key responsibilities

Effectively use the Nursing Process to gather data, plan, implement, and evaluate patient care.
Provide comprehensive care addressing physical, psycho‑social, educational, and safety needs of patients.
Communicate professionally with patients, families, and team members to ensure a quality experience.
Adapt quickly to changing unit needs and maintain personal professional development.

Required profile

High School diploma or equivalent.
Experience handling confidential information in a fast‑paced environment with minimal supervision.
Current LPN license in the state of practice.
Basic Life Support (BLS) certification from the American Heart Association.
Excellent organizational, time‑management, and judgment skills.
