Family Nurse Practitioner (FNP) - Outpatient Practice

About the role

An established outpatient family practice in Conyers, Georgia, is hiring a full-time Nurse Practitioner (FNP). The panel is already built and the schedule is full from day one, with a collaborating physician, dedicated billing and coding support, care management programs (CCM/RPM) running, and administrative staff handling paperwork. The practice serves a predominantly Black patient community in Rockdale County. The position is Monday to Friday with no nights or weekends.

Key responsibilities

• Provide outpatient primary care across the lifespan: acute visits, chronic disease management, preventive care and annual wellness visits
• See patients in an established panel, with no panel building or ramp-up period
• Document in eClinicalWorks (eCW) while billing and coding specialists review behind you
• Collaborate with an experienced supervising physician and a supportive back-office team

Required profile

• Active, unrestricted Georgia APRN license with FNP certification (or eligibility for new graduates completing certification)
• 2+ years of outpatient or primary care experience preferred; strong new graduates are welcome
• Experience with chronic disease management and EMR documentation
• A clinician who wants to know their patients rather than churn through them
