Evening RN Case Manager - Hospice

About the role

The Care Team, in partnership with Traditions Health, is seeking a second-shift RN Case Manager to join its growing hospice team in Peoria. The role administers skilled nursing care for clients of all ages in their place of residence, coordinates care with the interdisciplinary team, patients and their families and a referring agency, and takes responsibility for coordinating care. The position is Monday to Friday, 8-hour second-shift hours.

Key responsibilities

• Complete initial and ongoing assessments of physical, psychosocial and environmental needs, including OASIS or other assessments, and regularly re-evaluate nursing needs and care outcomes
• Develop, initiate and revise the plan of care, refer to other services and plan discharge
• Provide services requiring substantial nursing skill, counsel patients and families, and use infection control measures
• Coordinate services, inform the physician of changes in condition, and monitor cases for third-party payer compliance
• Prepare clinical and progress notes and participate in in-service programs
• Supervise, teach and direct other nursing personnel, assign home health aides, and supervise LPNs/LVNs and paraprofessionals
• Perform on-call duties and communicate effectively with patients, families and team members

Required profile

• Diploma, Associate or Baccalaureate degree from an accredited school of nursing
• Current state Registered Nurse license and current driver's license
• One year of RN experience in a clinical care setting; home health experience preferred
• Reliable transportation, valid driver's license and auto insurance
• Able to work flexible hours, travel locally, and handle prolonged standing, lifting up to 50 lbs and exposure to blood and bodily fluids

Required skills

• Nursing skills according to generally accepted standards of practice
• Good interpersonal skills and proof of current CPR certification
