Entitlements Case Manager – Part‑Time (W‑2 or 1099)

About the role

This part‑time Entitlements Case Manager position can be taken as a W‑2 employee or as an independent 1099 contractor. You will serve clients primarily located in community‑based group homes across two affiliated behavioral health entities, providing comprehensive entitlement and benefits support.

Key responsibilities

• Schedule and facilitate group and individual case‑management sessions for assigned clients.
• Conduct entitlement assessments and complete all required documentation within service windows.
• Assist clients with applications for identification, income support, healthcare, nutrition, housing, employment, education and other stabilizing resources.
• Coordinate service delivery across Lighthouse Behavioral Health Center and Community Wellness Outpatient Mental Health Center, including telehealth when appropriate.

Required profile

• Licensed Entitlements or Social Work professional with experience in case management.
• Ability to work approximately 40 hours per week, split evenly between the two entities.
• Willingness to travel to designated community sites in Baltimore and to conduct services onsite or via approved telehealth locations.
