Entitlements Case Manager (Part‑Time)

About the role

The Entitlements Case Manager provides comprehensive case management services to clients living in community‑based group homes in Baltimore. The position can be performed as a W‑2 employee or as a 1099 independent contractor, with roughly 40 hours per week split between two affiliated behavioral health entities.

Key responsibilities

• Schedule and facilitate group and individual case‑management sessions for assigned clients.
• Assist clients in applying for, maintaining, and renewing eligibility for a wide range of benefits, including income support, healthcare, housing, nutrition, and employment services.
• Conduct assessments, complete documentation, and coordinate services across Lighthouse Behavioral Health Center and Community Wellness Outpatient Mental Health Center.
• Provide services onsite at designated community locations or via telehealth when both provider and client are at approved sites.

Required profile

• Licensed Entitlements or Social Services Case Manager with experience serving community‑based populations.
• Ability to work part‑time (approximately 40 hours/week) across two entities and manage a caseload of 29 clients.
• Willingness to work onsite at specified Baltimore addresses and comply with each organization’s contractual and compliance requirements.
