Entitlements Case Manager – Part-Time (W‑2 or 1099)

About the role

The Entitlements Case Manager provides comprehensive case‑management services to clients living in community‑based group homes, coordinating benefits, documentation, and support across two affiliated behavioral health entities.

Key responsibilities

• Schedule and facilitate group and individual case‑management sessions for clients residing in community‑based group homes.
• Conduct comprehensive entitlement assessments and complete all required documentation within designated service periods.
• Assist clients in obtaining and maintaining identification, legal documents, and immigration paperwork.
• Support clients in applying for income, financial, healthcare, nutrition, housing, employment, education, and other stabilizing benefits.

Required profile

• Licensed Entitlements Case Manager with experience serving community‑based populations.
• Ability to work approximately 40 hours per week across two affiliated organizations, meeting scheduled service times.
• Compliance with professional licensing requirements and ability to maintain separate contracts for each entity.
