Entitlements Case Manager – Part-Time (W2 or 1099)

About the role

The Entitlements Case Manager provides comprehensive case management services to clients residing primarily in community‑based group homes. The role can be performed as a W‑2 employee or as an independent 1099 contractor, with responsibilities spanning two affiliated behavioral health organizations.

Key responsibilities

• Schedule and facilitate group and individual case management sessions for clients in community‑based group homes.
• Conduct comprehensive entitlement assessments and assist clients in applying for benefits such as SSI, Medicaid, SNAP, housing vouchers, and employment programs.
• Maintain accurate client documentation and coordinate services across Lighthouse Behavioral Health Center and Community Wellness Outpatient Mental Health Center.

Required profile

• Licensed and qualified Entitlements Case Manager with experience serving clients in community settings.
• Ability to work approximately 40 hours per week, split between two affiliated organizations.
• Must meet independent contractor classification requirements if selecting the 1099 option.
