Entitlements Case Manager – Part-Time (W-2 or 1099)

About the role

We are seeking a licensed Entitlements Case Manager to provide case management services to clients residing in community‑based group homes in Baltimore. The position can be taken as a W‑2 employee or as an independent 1099 contractor, split between Lighthouse Behavioral Health Center and Community Wellness Outpatient Mental Health Center.

Key responsibilities

• Schedule and facilitate group and individual case management sessions, conduct assessments, and complete client documentation.
• Assist clients in applying for and maintaining eligibility for benefits such as ID documents, income support, Medicaid, SNAP, housing vouchers, employment services, and education assistance.

Required profile

• Qualified and licensed Entitlements Case Manager with ability to work approximately 40 hours per week across two organizations.
• Willingness to serve clients at community‑based meeting rooms, main office, or via telehealth, and to manage separate contracts/invoicing for each entity.
