Entitlements Case Manager 3 – Part-Time (W‑2 or 1099)

About the role
The Entitlements Case Manager provides comprehensive case management services to clients living in community‑based group homes across Baltimore. The role can be performed as a W‑2 employee or as an independent 1099 contractor, with a part‑time schedule of roughly 40 hours per week split between two affiliated behavioral health entities.
Key responsibilities
- Schedule and facilitate group and individual case‑management sessions for assigned clients.
- Conduct entitlement assessments and complete all required documentation within service windows.
- Assist clients with obtaining and maintaining identification, legal documents, and immigration paperwork.
- Guide clients through applications for income supports such as SSI, SSDI, TCA, and veterans benefits.
- Coordinate healthcare enrollment, including Medicaid, Medicare, and managed‑care options.
- Support housing stability through public housing, Section 8, emergency rental assistance, and transitional housing referrals.
Required profile
- Licensed Entitlements or Social Services Case Manager with experience serving adults in group‑home settings.
- Ability to manage a caseload of approximately 29 Category A and other clients across multiple locations.
- Willingness to work onsite at designated Baltimore addresses and to travel between sites as needed.
- Capacity to invoice and manage separate contracts if choosing the 1099 arrangement.
