Clinical Pharmacist – Medication Therapy Management (Remote)

About the role

The Clinical Pharmacist will advance the quality, safety, and effectiveness of drug therapy for Healthspring Medicare members. Working remotely, you will join a multidisciplinary team to perform comprehensive medication reviews for high‑risk populations.

Key responsibilities

• Conduct comprehensive medication reviews (CMRs) including drug interaction assessment, therapeutic duplication identification, and renal/hepatic dosing adjustments.
• Address evidence‑based gaps in care and support STAR‑related pharmacy initiatives.
• Identify and resolve barriers to medication affordability and adherence.
• Participate in phone queue operations and manage case referrals, including narcotics programs.

Required profile

• Active, unrestricted Pharmacist license in the state of residence.
• Doctor of Pharmacy (PharmD) degree.
• Minimum three years of experience in Managed Care, Medicare or Medicaid settings (preferred).
• Residency and/or Board Certification (preferred/required).
• Bilingual proficiency in English and Spanish (preferred).
• Strong clinical medication knowledge and ability to apply evidence‑based guidelines.

Required skills

• Medication Therapy Management (MTM) experience.
• Familiarity with MTM platforms such as Adhere Health’s Optimize.
• Advanced proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook).
• Excellent verbal and written communication.
• Strong analytical, problem‑solving, and clinical decision‑making abilities.
