Clinical Pharmacist – Medication Therapy Management

About the role

We are seeking a Clinical Pharmacist to join a leading healthcare organization that provides personalized medication management services. The role focuses on empowering patients through comprehensive medication reviews, ensuring safe and effective use of their prescriptions.

Key responsibilities

• Conduct outbound calls to patients and perform detailed medication reviews.
• Provide in‑depth consultations on medication use following clinical guidelines.
• Accurately record prescription, diagnosis, and recommendation data in a web‑based application.
• Manage medication therapy services and offer guidance to improve adherence and safety.
• Develop and maintain standardized patient documentation.
• Participate in ongoing training to enhance clinical knowledge and skills.

Required profile

• Active Pharmacist’s License for the State of Florida (proof required).
• Minimum 4 years of experience in clinical medication therapy management.
• Knowledge of quality measures related to medication use and patient safety.
• Strong command of pharmaceutical and medical terminology.
• Fluent in written and spoken English with excellent communication skills.
• Highly organized, detail‑oriented, and able to handle confidential patient information.

Required skills

• Proficiency with customer relationship management (CRM) software.
