Chronic Care Management Assistant – Medical Hold Specialist

About the role

The Chronic Care Management (CCM) Assistant – Medical Hold Specialist supports Carda Health’s remote cardiac and pulmonary rehabilitation programs by ensuring accurate, HIPAA‑compliant patient data and conducting clinical checks.

Key responsibilities

• Maintain and update patient medical records in EMR systems, ensuring privacy and HIPAA compliance.
• Collect, organize, and review comprehensive past medical histories to identify contraindications for rehab programs.
• Perform medication reconciliation using knowledge of cardiac and pulmonary drugs.
• Respond to provider inquiries and facilitate communication between patients and physicians.
• Index and cross‑reference files for efficient retrieval and support general administrative tasks.

Required profile

• Bachelor’s degree in Clinical Exercise Physiology or Exercise Science.
• Minimum two years of experience in a medical office, preferably cardiac/pulmonary rehab.
• Strong attention to detail, organizational and problem‑solving abilities.
• Familiarity with HIPAA regulations and commitment to patient confidentiality.

• Knowledge of medical terminology and documentation standards.
