Case Worker – Full‑time, Miami, FL

About the role

The University of Miami/UHealth Department of Pediatrics seeks a full‑time Case Worker to support patients and families in Miami, FL. The role focuses on removing barriers to care, coordinating treatment plans, and ensuring access to insurance and social service programs.

Key responsibilities

• Conduct patient evaluations to develop effective treatment plans and monitor implementation.
• Enroll patients in insurance, Medicaid, Ryan White, ADAP and other low‑cost social service programs.
• Identify and resolve barriers such as transportation, childcare, cultural beliefs, and insurance issues.
• Provide medication adherence education, secondary prevention, and lifestyle management.
• Facilitate communication between patients and providers and link families to clinical trial opportunities.
• Travel to homes, daycares and other settings to conduct Individual Family Support Plan meetings.
• Collaborate with interdisciplinary teams and community agencies to secure early‑intervention and support services.
• Maintain confidential records in compliance with HIPAA and departmental guidelines.

Required profile

• Bachelor’s degree in psychology, social work, health education, early childhood, child development or related field.
• At least one year of case‑management or counseling experience with children, families or individuals with special needs.
• Proficiency with data and database systems; strong attention to detail.
• Ability to analyze, prioritize and meet deadlines under pressure.
• Comfort working evenings, nights or weekends as needed.
