Case Worker – Full-time, Pediatric Early Steps

About the role

The University of Miami Health System’s Department of Pediatrics – Early Steps is seeking a full‑time Case Worker 1 to support children and families in accessing and maintaining health‑care services. The role involves evaluating patients, coordinating treatment plans, and removing barriers to care.

Key responsibilities

• Conduct patient evaluations to gather medical, mental health, and recovery information for treatment planning.
• Enroll patients in eligible insurance programs and low‑cost social services such as Medicaid, SSI, WIC, and Ryan White.
• Identify and resolve barriers to care, including transportation, childcare, cultural beliefs, and insurance issues.
• Facilitate patient‑provider communication and provide medication adherence education.
• Deliver secondary prevention, lifestyle management, and counseling on illness‑related issues.
• Provide information on clinical trial opportunities and connect families with research staff.
• Maintain up‑to‑date knowledge of community health resources and ensure confidentiality in compliance with HIPAA.
• Document actions, assist with record audits, and adhere to university policies.

Required profile

• Experience conducting patient assessments and developing care plans.
• Familiarity with public insurance programs and enrollment processes.
• Ability to identify and address practical barriers to health‑care access.
• Strong communication skills for interacting with patients, families, and interdisciplinary teams.
• Willingness to travel to homes, daycares, and other community settings as needed.
