Case Worker 1 – Full‑Time Position

About the role

The University of Miami/UHealth Department of Pediatrics is seeking a full‑time Case Worker 1 to support patients and families in Miami, FL. The role focuses on removing barriers to care, coordinating treatment plans, and ensuring patients receive appropriate social and health services.

Key responsibilities

• Conduct patient evaluations to develop effective treatment plans.
• Enroll patients in insurance programs and low‑cost social services such as Medicaid, SSI, and Ryan White.
• Identify and resolve barriers to care, including transportation, childcare, cultural beliefs, and insurance issues.
• Provide medication adherence education and secondary prevention counseling.
• Travel to homes, daycares, and community settings to conduct Individual Family Support Plan meetings.

Required profile

• Bachelor’s degree in psychology, social work, health education, early childhood, or related field.
• At least one year of case management or counseling experience with children, families, or individuals with special needs.
• Ability to navigate and use database systems for documentation and reporting.
• Strong attention to detail and ability to handle confidential information in compliance with HIPAA.
• Willingness to work evenings, nights, and weekends as needed.
