Case Manager Registered Nurse (Full-Time)

About the role

The University of Miami Health System is seeking a full‑time Case Manager Registered Nurse to coordinate interdisciplinary care from admission through discharge. The RN will act as a liaison among patients, families, clinicians, and insurers to ensure optimal outcomes and efficient resource use.

Key responsibilities

• Develop and monitor individualized care plans, ensuring seamless transition across the care continuum.
• Assess patient needs, identify risk factors, and create discharge plans that align with available resources.
• Facilitate insurance authorizations, regulatory notifications, and obtain required patient signatures.
• Educate patients and families on care progression and post‑discharge instructions.
• Collaborate with the medical team to evaluate and adjust care pathways for quality improvement.

Required profile

• BSN from an accredited nursing program.
• Active Florida Registered Nurse license and BLS certification.
• Minimum 2 years of nursing experience, including at least 1 year in hospital case management.
• Strong knowledge of patient assessment, medical terminology, and insurance reimbursement policies.
• Excellent written and verbal communication, problem‑solving, and interpersonal skills.
