Case Manager – PM Shift

About the role

The Case Manager supports the Clinical Coordinator in delivering high‑quality substance‑use disorder services. Working on a PM shift, you will manage a client caseload, develop treatment plans, and coordinate care in a fast‑paced behavioral health setting.

Key responsibilities

• Develop and document individual treatment plans, monitor client progress, and ensure compliance with regulatory requirements.
• Maintain a caseload, complete progress notes, evaluations, and discharge planning in a timely manner.
• Provide individual and group interventions, including crisis management as needed.
• Facilitate coordination of care, case reviews, weekly progress updates, and after‑care services.
• Assist clients with applications for government assistance, probation/court obligations, and referrals to external agencies.

Required profile

• High school diploma or GED; 1–2 years of related experience in addiction or behavioral health.
• Current CPR/First Aid/Narcan certification, negative TB test, and valid fingerprint clearance card.
• Clean motor vehicle record preferred; successful background and Adult Protective Services checks required.
• Working knowledge of addiction, recovery, and twelve‑step programs.

Required skills

• Proficiency with Microsoft Office applications.
• Experience using electronic medical record (EMR) systems.
