Caregiver Coach

About the role

Ceresti Health is looking for a compassionate Dementia Caregiver Coach to provide remote support, coaching, and education to family caregivers of individuals living with dementia. This role is central to our innovative Caregiver Support Program, aimed at improving outcomes and reducing hospitalizations.

Key responsibilities

• Deliver person‑centered coaching to family caregivers, tailored to their unique challenges.
• Use Ceresti’s proprietary technology platform to manage workflow, track progress, and document interactions.
• Educate caregivers on dementia processes, behavioral symptoms, safety strategies, and stress‑management techniques.
• Guide caregivers through emotionally complex scenarios such as end‑of‑life conversations and transitions to facility care.
• Assist caregivers with care planning, appointment preparation, resource navigation, and communication with healthcare providers.

Required profile

• 2+ years of professional experience working with older adults or persons living with dementia.
• Direct caregiving experience (personal or professional) with individuals with cognitive impairment.
• Fluency in English and eligibility to work in the U.S.
• Ability to work independently in a fully remote setting.

Required skills

• Proficiency with Mac/IOS platforms.
• Experience working in HIPAA‑compliant environments.
