Cardiac and Pulmonary Rehab Telehealth Physician

About the role

Carda Health is seeking a board‑certified Cardiologist or Pulmonologist to join its virtual heart and lung platform as an independent contractor. You will provide telehealth evaluations and ongoing oversight for patients enrolled in cardiac and pulmonary rehabilitation programs.

Key responsibilities

• Conduct asynchronous or synchronous patient visits, including health histories, diagnoses, and referrals to rehab programs.
• Develop and update monthly treatment plans, coordinate with remote patient monitoring (RPM) services, and document all interactions in the EMR.
• Collaborate with multidisciplinary care teams to ensure continuity of care and participate in quality‑assurance meetings.

Required profile

• Board‑certified in Cardiology, Pulmonology, Internal Medicine or Family Medicine.
• Licensed in multiple states (preferably five or more, with priority states listed).
• Clean credentialing record, not opted out of Medicare, and able to complete credentialing promptly.

Required skills

• Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification.
• Proficiency with electronic medical record (EMR) systems for documentation and care coordination.
