Bilingual Clinical Nurse Coach (RN)

About the role

Renalogic is seeking a Bilingual Clinical Nurse Coach to provide remote health coaching to members at risk for or living with chronic kidney disease. The role involves developing individualized care plans, engaging members in both English and Spanish, and supporting them to avoid dialysis.

Key responsibilities

• Establish and maintain contact with members via video, phone, text, and email in English and Spanish.
• Develop individualized member care plans following NCQA guidelines and assess health status and care coordination needs.
• Collaborate with members’ healthcare providers and use motivational interviewing and the Triple Aim framework to empower members.
• Document and share member information in compliance with HIPAA and meet performance metrics for engagement and retention.

Required profile

• Active RN license in good standing, willing to obtain licensure in additional states.
• Minimum five years of RN experience, preferably with chronic conditions or CKD care.
• Fluent native‑level English and Spanish (read, write, speak).
• Willingness to work two late shifts (11 am‑8 pm PST) and three day shifts (8 am‑5 pm PST) weekly.
• Ability to travel occasionally for corporate events and conferences.

Required skills

• Proficient with Microsoft Office and related computer applications.
• Experience creating care plans for chronic disease patients.
• Strong autonomous, self‑starter attitude and ability to manage ambiguity.
