Behavioral Health Case Manager – Substance Use Disorder

About the role

Renewal Falls Recovery Center is seeking a compassionate, organized, and resourceful Substance Use Disorder Case Manager to support patients in Cedar Falls, Iowa. The role focuses on addressing real‑life barriers such as housing, transportation, employment, and healthcare to promote long‑term recovery.

Key responsibilities

• Conduct comprehensive case management assessments to identify patient needs, strengths, and barriers.
• Develop individualized case management goals in collaboration with patients and the interdisciplinary treatment team.
• Initiate discharge and continuing‑care planning early in the treatment process.
• Coordinate referrals and connections to community resources, including housing, transportation, employment, education, insurance, and healthcare services.
• Assist patients in accessing social‑service supports and benefits.
