Behavioral Health Care Management Specialist

About the role

HCSC is hiring a Behavioral Health Care Management Specialist (position 743) to conduct care management and health education programs for customers on government health care programs. The role represents the company to its members and involves gathering, analyzing and providing data for regulatory reports. It is a union role and may be performed remotely from anywhere in the continental United States, excluding California, New York, Alaska and Hawaii.

Key responsibilities

• Conduct care management and health education programs for members on government health care programs
• Gather, analyze and provide data for regulatory reports
• Represent the company to its members
• Visit members in the Chicago area

Required profile

• Registered Nurse (RN) with 2 years of direct clinical care to consumers in a clinical setting, or an unrestricted licensed counselor, social worker, marriage and family therapist or mental health counselor in the state of operations (with reciprocity)
• Plus 3 years of wellness or managed care experience presenting clinical issues with members and physicians
• Experience handling medical management and health education programs independently
• Knowledge of the health and wellness marketplace and employer trends
• Verbal and written communication skills, analytical experience including medical data analysis, PC proficiency (Word, Excel, PowerPoint, databases, web applications) and willingness to travel within the assigned territory
• PREFERRED: bilingual English/Spanish, patient education and condition management experience, experience with complex or catastrophic cases, case management or population management experience, and nationally recognized healthcare certification
