Social Services Program Coordinator II – HIV Linkage

About the role

The State of Georgia’s County Public Health department is seeking a Social Services Program Coordinator II to serve as the EHE Linkage Coordinator for Cobb and Douglas Counties. Under general supervision, you will promote and protect the health of residents by linking individuals living with HIV to medical care, treatment, and support services.

Key responsibilities

• Implement the Antiretroviral Treatment and Access to Services (ARTAS) strategy for newly diagnosed or out‑of‑care individuals.
• Coordinate timely referrals and follow‑up for HIV care, partner services, STI, hepatitis, TB screening, mental health, and substance‑use treatment.
• Promote routine and opt‑out HIV testing, self‑testing, and other prevention strategies in healthcare and community settings.
• Perform HIV testing, counseling, and outreach for high‑risk community residents.
• Ensure timely disease‑intervention strategies to reduce HIV and STD prevalence.
• Maintain documentation and reporting required for grant‑funded HIV Prevention Program.

Required profile

• Bachelor’s degree in a related field from an accredited institution.
• At least two years of relevant experience or equivalent experience at the lower level Social Services Program Coordinator I.
• Knowledge of HIV/AIDS care processes and Ryan White eligibility requirements.
