RN Navigator – Oncology Clinic

About the role

The RN Navigator in the Oncology Clinic serves as the primary liaison among patients, caregivers, healthcare providers, and the multidisciplinary team. This position ensures smooth transitions across care settings, supports discharge planning, and promotes cost‑effective, high‑quality outcomes.

Key responsibilities

• Provide continuity of care by coordinating transitions between acute, post‑acute, and outpatient settings.
• Develop and maintain relationships with patients and their multidisciplinary teams to navigate treatment and follow‑up.
• Manage high‑risk, complex cases to minimize readmissions and coordinate post‑discharge plans.
• Collaborate closely with physicians and other team members to communicate and update patient‑centered care plans.
• Identify barriers to care and recommend process improvements for patient navigation.
• Act as patient advocate, adapting communication style to respect privacy and individual needs.

Required profile

• Graduate of an accredited nursing school; Associate’s or Bachelor’s degree preferred.
• Minimum 2 years of nursing experience.
• Current RN license in the state of practice and Basic Life Support (BLS) certification.
• Proficiency with computers, software, and web‑based applications.
• Effective verbal and written communication skills and strong conflict‑resolution abilities.
• Flexibility to work varied schedules, including weekends, holidays, on‑call, and travel between facilities.
