RN Navigator – OB/GYN Clinic (Monroe, LA)

About the role

This position serves as the liaison and communicator among patients, caregivers, healthcare providers, and the multidisciplinary team, including post‑acute care and third‑party payers. The RN Navigator discusses alternative care options, assists with discharge planning, and helps ensure smooth transitions across the care continuum.

Key responsibilities

• Provide continuity of care by coordinating smooth transitions between care settings.
• Develop relationships with patients and multidisciplinary teams to navigate subsequent treatment and follow‑up.
• Manage high‑risk, complex patient cases to minimize readmissions.
• Collaborate closely with physicians and the care team to maintain an up‑to‑date, patient‑centered care plan.
• Prepare, execute, and reinforce post‑discharge care plans.
• Identify barriers to care and recommend process improvements.
• Work with leadership to review and enhance clinical experiences for referred patients and physicians.
• Adapt communication style to respect patient privacy and preferences.

Required profile

• Graduate of an accredited nursing school (required).
• Minimum 2 years of nursing experience.
• Current RN license in the state of practice.
• Basic Life Support (BLS) certification from the American Heart Association.
• Effective verbal and written communication skills.
• Ability to work a flexible schedule, including weekends, holidays, on‑call, and travel between facilities.
