RN Medical Case Manager (Travel Required)

About the role

We are seeking a Registered Nurse (RN) to join our team as a Medical Case Manager. The role is primarily remote, with travel up to a two‑hour drive to attend patient appointments and coordinate care across Michigan.

Key responsibilities

• Lead a multidisciplinary team—including social workers, dietitians, pharmacists, and medical directors—to deliver holistic, person‑centred care.
• Assess members’ health, psychosocial needs, cultural preferences, and support systems.
• Develop, implement, monitor, and evaluate individualized care plans.
• Arrange community resources, mental‑health services, substance‑abuse support, and financial assistance as needed.
• Advocate for members, provide health‑literacy education, and ensure smooth transitions between care settings.
• Document all interactions accurately and track progress toward care goals.

Required profile

• Active RN license in the State of Michigan.
• CCM certification highly preferred.
• Willingness to travel within a two‑hour radius for patient appointments.
• Strong communication skills across phone, email, text, and online messaging platforms.
