RN III Nurse Home Visitor

About the role

The RN III Nurse Home Visitor provides comprehensive nursing care and support to women and families enrolled in the Nurse‑Family Partnership (NFP) program. Working under general supervision, the RN conducts home visits, promotes maternal and child health, and supports clinic operations.

Key responsibilities

• Deliver nursing services to up to 30 families participating in the NFP program, including home visits and health education.
• Maintain accurate client records, evaluate progress, and report data to the program team.
• Manage clinic inventory, requisition medical supplies, and ensure safe service delivery.
• Participate in team meetings, community coalitions, and outreach activities to promote the NFP program.
• Complete required trainings, engage in professional development, and perform other duties as assigned.

Required profile

• Associate degree in Nursing (or equivalent) with four years of related nursing experience.
• Active Registered Nurse license in Texas, in good standing.
• CPR certification (required within 30 days of hire).
• Strong commitment to reflective practice and adherence to professional nursing standards.
