Program Aide – Root Cause Analysis Specialist

About the role

The Program Aide will support the Patient Safety Organization by reviewing Root Cause Analysis (RCA) documentation submitted by healthcare facilities. This part‑time, onsite position focuses on improving patient safety outcomes through rigorous analysis and feedback.

Key responsibilities

• Conduct comprehensive quality reviews of RCA reports to identify gaps in documentation, causal analysis, and action planning.
• Provide consultative feedback to facility RCA teams to enhance event analysis and corrective action development.
• Ensure RCA submissions meet established quality standards based on nationally recognized methodologies.
• Support the organization’s Patient Safety Organization (PSO) functions and maintain compliance with Joint Commission sentinel event requirements.

Required profile

• Bachelor’s degree in healthcare administration, nursing, public health, quality improvement, or a related field.
• At least 3 years of experience participating in Root Cause Analyses within a healthcare setting.
• Strong knowledge of patient safety principles and systems‑based error prevention.

Required skills

• Root Cause Analysis methodology, including the Five Rules of Causation and Action.
• Human factors engineering principles.
• Familiarity with VHA National Center for Patient Safety RCA methodology or NPSF RCA2 methodology.
