Physical Therapist - Travel Contract

About the role

This travel position offers a Physical Therapist the chance to work in home‑health settings across Palestine, TX and surrounding areas while enjoying the lifestyle of East Texas. You will operate as an independent contractor delivering skilled therapy services to patients in their homes.

Key responsibilities

• Conduct home‑health physical therapy evaluations and start‑of‑care visits.
• Perform 30‑day reassessments and ongoing treatment visits.
• Complete discipline and agency discharges according to standards.
• Document all care accurately in the agency’s system.
• Travel between patient locations with mileage reimbursement.

Required profile

• Bachelor’s, Master’s or Doctoral degree in Physical Therapy from an APTA‑approved program.
• Active Texas Physical Therapy license at start of assignment.
• Home‑health experience including evaluations, reassessments, and treatment visits.
• Ability to work independently as a contractor and meet agency documentation requirements.

Required skills

• BLS Certification (AHA or ARC).
• Familiarity with the HCHB electronic medical record system.
• Proficiency in home‑health physical therapy assessments and documentation.
