Pharmacist – Long-Term Care Pharmacy

About the role

Guardian Pharmacy of Minnesota is seeking an experienced pharmacist to join its fast‑growing long‑term care pharmacy team in Eden Prairie. You will play a critical role in ensuring safe, accurate, and timely medication dispensing for senior and complex‑care patients.

Key responsibilities

• Prepare and dispense medications, including order entry, filling, packaging, and final verification.
• Review prescriptions for legality, feasibility, allergies, drug interactions, and refill limits.
• Interpret orders, exercising professional judgment to protect patient safety.
• Maintain compliance with Federal and State Board of Pharmacy regulations and compounding guidelines.
• Transfer prescriptions when necessary and respond to inquiries from patients, clinicians, and staff.
• Monitor inventory levels, place orders, and identify cost‑effective alternatives for exception items.
• Communicate stock shortages and expected fulfillment dates to customers or facilities.
