Pharmacist Consultant

About the role

PharMerica is seeking a remote Pharmacist Consultant to provide clinical pharmacy services for long‑term care and senior living facilities. The consultant will evaluate medication regimens, support prescribers, and help facilities meet regulatory requirements while optimizing resident outcomes.

Key responsibilities

• Review physician medication orders for appropriateness, dosage, interactions, route, and stability.
• Make therapy recommendations to physicians, nursing staff, and administrators.
• Conduct monthly medication regimen reviews for assigned residents and document findings.
• Communicate recommendations and documentation to the Director of Nursing, administrators, and attending physicians.
• Stay current with federal and state pharmacy laws and long‑term care regulations.
• Collaborate with interdisciplinary care teams to improve medication safety and cost‑containment.

Required profile

• Bachelor’s degree from an accredited School of Pharmacy or a Doctor of Pharmacy (PharmD) degree.
• Active pharmacist license in the state(s) covering the assigned territories.
• Immunization certification from an accredited organization (e.g., APhA).
• Experience in long‑term care or senior living pharmacy settings is preferred.
