Part‑Time Health Care Worker – Community Integration Home

About the role

The State of Georgia is seeking a part‑time Health Care Worker to support residents at the Community Integration Home in Thomasville. Under general supervision, you will help individuals with daily living activities, promote skill development, and ensure a high‑quality care environment.

Key responsibilities

• Assist residents with personal care, meal preparation, and daily living tasks.
• Observe, monitor, and chart resident behavior and incidents.
• Provide basic training in personal, social, academic, or work skills.
• Arrange medical appointments and support residents in obtaining needed services.
• Develop and contribute to individual service/treatment plans.
• Offer counseling on behavioral issues and daily decision‑making.
• Perform housekeeping duties, including cleaning, laundry, and errands.
• Drive state‑owned vehicles to transport residents to appointments and activities.
• Maintain accurate records of resident progress and report changes to supervisors.

Required profile

• Ability to work under general supervision while providing day‑to‑day assistance to residents.
• Strong communication skills for interaction with mental‑health professionals and residents.
• Reliability in performing transportation, meal planning, and case‑management tasks.
• Willingness to participate in team reviews and contribute to service planning.

Required skills

• Basic computer skills.
