Nurse Practitioner – Value Based Care (Lithonia, GA)

About the role

Upperline Health is seeking a Nurse Practitioner to join its Lithonia, GA clinic. The role focuses on delivering value‑based, patient‑centered care for adults with chronic conditions, working alongside physicians and a multidisciplinary team in a regular weekday schedule.

Key responsibilities

• Develop and implement individualized care plans for patients with diabetes, COPD, CHF, dementia and other chronic illnesses.
• Conduct patient histories, physical examinations, and point‑of‑care diagnostics in the clinic or home setting.
• Provide medical treatments, prescribe medications, and coordinate follow‑up care within the scope of practice.
• Educate patients and families on medication management, nutrition, and preventive strategies.
• Communicate clinical information to physicians, pharmacists, care navigators and other team members.

Required profile

• Current Nurse Practitioner licensure and active state license.
• Experience in primary care, internal medicine, geriatric, home‑based, palliative or hospice care, preferably in a value‑based environment.
• Demonstrated compassion, attention to detail and ability to thrive in a collaborative, fast‑growing setting.
