Nurse Practitioner – Value-Based Care in East Cobb, GA

About the role

Upperline Health is seeking a Nurse Practitioner to join its Kennestone/East Cobb clinic. The role combines clinic‑based, home‑visit, and telehealth care for adults with chronic conditions, operating within a value‑based care model.

Key responsibilities

• Develop and implement individualized care plans for patients with diabetes, COPD, CHF, dementia and other chronic illnesses.
• Conduct patient histories, physical examinations, and diagnostic assessments in the clinic, at home, or via telehealth.
• Collaborate with physicians, pharmacists, dietitians, social workers and other team members to coordinate comprehensive treatment.
• Educate patients and families on medication management, nutrition, and preventive strategies.
• Communicate clinical information to patients’ primary physicians and the broader interdisciplinary team.

Required profile

• Experience in primary care, internal medicine, geriatric, home‑based, palliative or hospice care.
• Familiarity with value‑based care environments.
• Compassionate, detail‑oriented, and able to thrive in a collaborative, fast‑growing setting.
