Nurse Care Coordinator (RN) – Remote, Full-Time

About the role

The Nurse Care Coordinator (RN) is an integral member of Clarity Pediatrics' interdisciplinary care team, coordinating appointments, providing education, managing medication authorizations, and serving as the primary point of contact for families, providers, and pharmacies. This is a fully remote position with set clinic hours.

Key responsibilities

• Coordinate, schedule, and reschedule clinical appointments across the care team and manage referrals to specialists.
• Proactively follow up with families to reduce gaps in care and no‑show rates, and support care transitions.
• Provide evidence‑based education to families about chronic condition management and respond to inquiries via messaging and phone.
• Initiate and manage medication prior‑authorization requests, communicate with pharmacies, and track authorization status.
• Document clinical interactions accurately across multiple EHR and care‑management platforms.
• Participate in interdisciplinary case conferences, team huddles, and quality‑improvement meetings.

Required profile

• Active California Registered Nurse (RN) license in good standing.
• Proficiency with multiple technology platforms and electronic health record (EHR) systems.
• Strong organizational, communication, and interpersonal skills.
• Ability to manage competing priorities in a remote, virtual environment.
• Comfort working with diverse, medically and socially complex pediatric populations.
• Startup mindset and excitement about iterating new care models.
• Reliable internet connection and dedicated remote workspace.
• Bilingual English/Spanish.
