Licensed Master Social Worker – Transplant

About the role

The Transplant Licensed Social Worker joins the solid organ transplant multidisciplinary team at UAMS, providing comprehensive support to patients and families throughout all phases of care. This position requires a deep understanding of social work practice, state regulations, and federal requirements such as CMS Conditions of Participation and EMTALA.

Key responsibilities

• Collaborate with physicians, nurses, and allied health professionals to coordinate patient management from evaluation through post‑transplant follow‑up.
• Conduct assessments of psychological, financial, and discharge‑planning needs for diverse age groups, including neonatal, pediatric, adolescent, adult, and geriatric populations.
• Provide counseling, education, and referrals to community resources, ensuring patients receive appropriate levels of care and support.
• Maintain compliance with UAMS mission, standards, and regulatory guidelines, documenting all interventions accurately.

Required profile

• Master’s degree in Social Work.
• Licensed Master Social Worker (LMSW) in Arkansas by the date of hire.
• Experience in case management and working in a medical setting (preferred).
• Certified Case Manager (CCM) or American Case Management (ACM) certification (preferred).
• Experience serving transplant patient populations (preferred).
