Licensed Behavioral Health Consultant

About the role

Evergreen Family Medicine seeks a Licensed Behavioral Health Consultant to deliver brief, evidence‑based interventions within its primary‑care clinics in Roseburg. The role blends on‑site, telehealth, and community‑based services to support patients with psychiatric and psychosocial concerns.

Key responsibilities

• Provide on‑site and telehealth behavioral health services to patients referred by primary‑care providers.
• Maintain an accessible schedule for immediate provider consultations and respond promptly to calls.
• Conduct initial assessments and brief interventions focused on the referral issue.
• Apply brief cognitive‑behavioral therapy techniques and other evidence‑based practices.
• Deliver crisis evaluation and intervention as needed.
• Collect and use screening tool data to guide patient care.
• Maintain up‑to‑date resource lists and facilitate appropriate referrals.
• Collaborate with clinic leadership to develop program services and support groups.

Required profile

• Master’s degree in psychology, social work, counseling, or a related field.
• Minimum two years of clinical service delivery for individuals with mental or behavioral health needs.
• Current Oregon licensure (e.g., LCSW, LPC, LMFT, or psychologist license) or eligibility for independent licensure.
• Experience in primary‑care or medical settings preferred.
