Housing Navigator – Support for Persons Experiencing Homelessness

About the role

The Housing Navigator provides housing‑focused supportive services to persons experiencing homelessness (PEH). Working under the Housing Navigation Supervisor, you will help participants secure permanent housing and ensure a safe discharge plan from treatment settings.

Key responsibilities

• Conduct comprehensive clinical assessments and develop individualized housing and support plans.
• Coordinate documentation, maintain HMIS records, and prepare housing applications.
• Build relationships with agencies, landlords, and service providers to secure temporary and permanent housing.
• Perform home visits, provide crisis intervention, and deliver trauma‑informed, harm‑reduction care.

Required profile

• High School Diploma or GED.
• At least 2 years experience in housing or related services with diverse populations.
• Minimum 1 year experience in housing navigation or case management.
• Valid California driver’s license, vehicle, and insurance.

Required skills

• Basic proficiency with Microsoft Word.
• Basic proficiency with Microsoft Excel.
• Basic proficiency with Microsoft Outlook.
• Experience using HMIS (Homeless Management Information System).
