Home Health LPN/LVN with $10,000 Sign-On Bonus

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based services. The Home Health Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) works under the supervision of a Registered Nurse to provide skilled nursing care to patients in their homes. The position is advertised with a $10,000 sign-on bonus for external candidates.

Key responsibilities

• Verify the plan of care with the RN before each visit
• Collaborate with the physician and RN to meet patient needs and notify them promptly of any change in condition
• Teach patients and caregivers self-care techniques
• Check vital signs and review medications, performing medication reconciliation at each visit
• Perform specialized procedures ordered by the physician, such as a foley catheter change
• Prepare equipment and materials for treatments using aseptic technique
• Complete documentation timely and accurately

Required profile

• Current LPN license for the state(s) of assignment
• Current CPR certification
• At least 1 year of LPN or LVN experience
• Valid driver's license, reliable transportation and liability insurance

What we offer

• $10,000 sign-on bonus for external candidates
• Total cash pay of $54,324 to $81,487 annually, with $31.34 to $47.01 per visit point and an hourly rate of $26.12 to $39.18
