Clinical Pharmacist (Full-Time, Variable Shift)

About the role

The Clinical Pharmacist at SVH delivers comprehensive pharmaceutical care across the lifespan, optimizing medication therapy, preventing disease, and promoting health and wellness. The role follows the American College of Clinical Pharmacy philosophy, blending caring orientation with specialized therapeutic expertise to achieve optimal patient outcomes.

Key responsibilities

• Review, interpret, and verify provider medication orders, providing recommendations and clinical interventions based on patient‑specific factors, safety, and cost‑effectiveness.
• Conduct pharmacy consults in accordance with all pharmacy policies and procedures.
• Assess patients' health problems, determine appropriateness of prescribed therapies, and intervene when necessary.
• Collaborate with interdisciplinary teams to support disease‑state management and health promotion.

Required profile

• Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD) from an accredited school.
• Successful completion of an ASHP‑accredited residency plus at least one year of hospital or relevant clinical experience, or equivalent clinical fellowship.
• Current pharmacist license (must be obtained within 90 days of hire).
• Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications within 90 days.
• Vaccine certification within one year and ASHP or equivalent clinical certification within two years.

Required skills

• Proficiency in medical terminology and medication nomenclature.
• Ability to apply mathematical concepts for data analysis and variance testing.
• Competence with spreadsheets, database software, and electronic medical record (EMR) systems.
• Strong ability to read, analyze, and interpret professional journals, technical procedures, and regulations.
• Excellent written communication for reports, business correspondence, and procedural documentation.
