About the role
The Case Manager will provide clinical case management services for the Prince George's County Department of Corrections under the supervision of the Rebirth Substance Use Treatment Program Clinical Coordinator.
Key responsibilities
- Complete program enrollment and screening/assessment tools.
- Develop person‑centered continuity of care plans and make appropriate referrals.
- Follow up with departmental team providers and community‑based agencies.
- Submit client status and progress reports as required.
- Maintain HIPAA‑compliant electronic case files for all clinical cases.
- Participate in regular clinical supervision and training on mental health and substance‑use issues.
Required profile
- Bachelor’s degree in Psychology, Criminology, Sociology, Public/Business Administration, Education, or a closely related field.
- Valid driver’s license.
