Ochsner Health is seeking a Manager to lead its Care Management team in Monroe, Louisiana. The role focuses on coordinating case and care managers, ensuring evidence‑based practices improve patient outcomes, and fostering collaboration across clinical and ancillary staff.

Key responsibilities

• Oversee daily department operations, maintaining appropriate staffing levels and assignments.
• Develop and maintain relationships with primary care physicians, physician advisors, and clinical teams to coordinate care and discharge planning.
• Research, select, and adapt best‑practice findings to enhance quality of patient care.
• Analyze data to identify process improvements and remove barriers for care managers.
• Interview, train, motivate, evaluate, and monitor staff performance.
• Ensure compliance with federal, state, and accreditation standards.

Required profile

• Registered Nurse diploma or Master’s Degree in Social Work.
• Minimum 3 years experience in case management, disease management, or utilization management.
• Current Louisiana RN license or Licensed Master Social Worker (LMSW); CCM certification preferred.
• Strong leadership, delegation, organizational, and time‑management abilities.
• Effective verbal and written communication skills for presenting information to diverse audiences.
