Advanced Practice Provider (NP/PA) – Street Medicine

About the role

Wellness Equity Alliance is expanding its street‑medicine program in Southern California. As an Advanced Practice Provider (NP or PA) you will deliver primary, mental health, substance‑use and infectious‑disease care to unsheltered individuals across the greater San Diego area.

Key responsibilities

• Provide comprehensive clinical assessments and treatment to homeless populations living in encampments, tents, vehicles, shelters or temporary housing.
• Collaborate with nurses, case managers and other multidisciplinary team members to develop and implement individualized care plans.
• Promote health education, disease prevention and health‑promotion activities within the community.
• Serve as the first point of contact to identify evident or underlying health issues and coordinate follow‑up services.
• Document care encounters and contribute to data‑driven population health analytics.

Required profile

• Current NP or PA licensure with experience serving special or underserved populations.
• Demonstrated experience in public‑health or community‑based settings, preferably with homeless or street‑medicine programs.
• Ability to work independently in mobile, non‑traditional environments.
• Commitment to trauma‑informed, culturally responsive care.

• Annualized compensation range $135,200–$166,400 (based on 1.0 FTE).
