PRN Social Worker (MSW) – Mon‑Fri 8am‑4:30pm

About the role

We are seeking a Master‑level Social Worker to provide psychosocial assessments, patient education, and crisis interventions for patients in our acute and chronic care settings. The position works on a PRN basis, Monday‑Friday, 8 am‑4:30 pm, and collaborates with a multidisciplinary team to develop and coordinate discharge plans.

Key responsibilities

• Conduct bio‑psychosocial assessments and interview patients and families.
• Identify community resources, make referrals, and coordinate complex discharge plans.
• Document assessments, referrals, and care plans in the medical record and communicate critical information to the care team.
• Participate in on‑call rotation and adapt communication style to diverse patient populations.

Required profile

• Master’s degree in Social Work (MSW) from a CSWE‑accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW); LMSW preferred or to be obtained within 6 months.
• No prior experience required; two years of medical‑setting experience preferred.
• Knowledge of common disease entities, medical terminology, and health‑care financing (Medicare, Medicaid, commercial plans).

Required skills

• Computer data‑entry and retrieval skills.
• Proficiency with Windows‑style applications and role‑specific software.
• Understanding of medical terminology and health‑care system codes.
• Familiarity with Medicare, Medicaid, and other public insurance programs.
