Primary Care Nurse Practitioner / Physician Assistant (Part‑Time, Weekends)

About the role

Infinite Medical P.C. is seeking a dedicated Primary Care Nurse Practitioner or Physician Assistant to join our team on a part‑time, weekend basis. You will deliver on‑site primary care to residents in skilled nursing and long‑term care facilities in Rome, NY, fostering meaningful patient relationships in a collaborative environment.

Key responsibilities

• Provide primary care services, including physical exams and medical history assessments, to residents.
• Diagnose and treat chronic and acute health conditions.
• Order, interpret, and act on diagnostic tests and laboratory results.
• Develop, adjust, and monitor individualized care plans.
• Prescribe medications and evaluate treatment effectiveness.
• Collaborate with facility staff and interdisciplinary teams.
• Educate patients and families about treatment options and care goals.
• Maintain a safe, respectful, and supportive environment for residents.

Required profile

• Active, unrestricted NP, PA, or Physician license in New York, in good standing.
• Minimum six months experience in long‑term care or skilled nursing facilities.
• Strong communication, clinical, and critical‑thinking abilities.
• Passion for elder care and person‑centered healthcare.
• Physical ability to lift at least 20 lb and stand or walk for 6‑8 hours per day.
