Pediatric Ambulatory Clinical Pharmacist

About the role

The Pediatric Ambulatory Clinical Pharmacist will manage medication therapy for assigned pediatric patients across ambulatory settings, ensuring safe, effective, and cost‑efficient care. The role involves direct patient interaction, education, and collaboration with multidisciplinary teams to optimize outcomes.

Key responsibilities

• Provide pharmacokinetic consultation and evaluate drug selection and dosing using laboratory data.
• Monitor and evaluate medication therapy, perform medication reconciliation, and educate patients and healthcare providers.
• Develop order sets, pathways, best practices, and guidelines for the pediatric ambulatory population.
• Mentor pharmacy and medical residents, students, and new staff, and contribute to publications.

Required profile

• Bachelor of Science in Pharmacy (BS) required; PharmD preferred.
• Current state pharmacy license.
• Completion of a PGY1 Pharmacy Practice Residency (or three years related specialty experience); PGY2 residency preferred.
• Effective verbal and written communication and leadership/teaching abilities.

Required skills

• Pharmacokinetic consultation and dosing calculations.
• Medication reconciliation and medication monitoring.
• Proficiency with Windows‑style applications and data entry.
• Use of laboratory data to assess drug therapy.
