Part-Time Physical Therapist

About the role

Habitat Health’s PACE program in Sacramento is looking for a part‑time Physical Therapist to provide comprehensive evaluation and treatment for older adults. The role involves delivering care both at the center and in participants’ homes, creating individualized therapy plans, and educating participants and caregivers on exercises and durable medical equipment (DME) use.

Key responsibilities

• Conduct physical therapy assessments and develop treatment plans across acute, restorative, and maintenance levels.
• Provide hands‑on therapy in the PACE center and make home visits as needed.
• Document all clinical activities, participant responses, and care coordination in the electronic health record.
• Participate in the Interdisciplinary Team, contributing insights for whole‑person care planning.
• Serve as a subject‑matter expert for program standardization, team education, and quality audits.
• Manage DME inventory and ensure compliance with safety and infection‑control standards.

Required profile

• Current state licensure as a Physical Therapist.
• Experience delivering therapy to older adult populations, preferably in a PACE or similar setting.
• Strong communication skills for interacting with participants, caregivers, and interdisciplinary team members.
• Ability to work independently during home visits and collaboratively within a team environment.
