Mobile Nurse Practitioner / Physician Assistant – Wound Care

About the role

West Coast Wound & Skin Care is seeking a licensed Nurse Practitioner or Physician Assistant to deliver advanced mobile wound‑care services to patients in their homes or assisted‑living facilities across the Philadelphia area. You will travel locally, providing hands‑on treatment, education, and follow‑up care while working within a supportive, fast‑growing team.

Key responsibilities

• Select, order, and perform diagnostic tests and procedures to guide patient treatment.
• Perform wound‑care procedures, including debridement and application of topical products.
• Define and document patient diagnoses and treatment plans.
• Educate patients and caregivers on wound management and overall health.
• Maintain accurate, timely notes in patient charts, including follow‑up documentation.

Required profile

• Current state license as a Nurse Practitioner or Physician Assistant in Pennsylvania.
• CPR certification.
• Valid driver’s license and minimum state‑required liability auto insurance.
• Excellent communication and interpersonal skills.
• Willingness to travel locally to patient homes and ALFs.
