Full‑Time Social Worker (MSW) with Sign‑On Bonus

About the role

Ochsner Health is seeking a dedicated Social Worker with an MSW to join its multidisciplinary team. The role involves conducting biopsychosocial assessments, providing patient education, and coordinating complex discharge plans. You will work directly with patients, families, and community resources to address emotional, social, cultural, and spiritual needs.

Key responsibilities

• Conduct patient assessment interviews, provide referrals, and coordinate discharge plans.
• Identify and connect patients and families with community resources and support services.
• Maintain accurate patient records, document interventions, and communicate critical information to the medical team.
• Participate in on‑call rotation and adapt communication style to diverse patient populations.
• Stay current with professional certifications and pursue continuing education.

Required profile

• Master’s degree in Social Work (MSW) from a CSWE‑accredited program.
• Certified Social Worker (CSW) or Registered Social Worker (RSW); LMSW preferred or to be obtained within six months.
• Preferred two years of experience in a medical or hospital setting (not mandatory).
• Knowledge of Medicare, Medicaid, and other health‑care financing programs.

Required skills

• Computer data‑entry proficiency and ability to retrieve job‑related information.
• Strong command of Windows operating system and related software applications.
• Familiarity with medical terminology and electronic health‑record systems.
